Organizations that use their data to learn and improve are more likely to succeed in value-based contracts. With an initial focus on success in the Primary Care First - Serious Illness Population (PCF-SIP) program, the Performance Collaborative will help teams:
1. Benefit from advanced data science capabilities
2. Learn how to integrate data-driven insights into clinical workflows
3. Share best practices with peer organizations
4. Achieve superior performance in the PCF-SIP model
5. Develop the skills required to negotiate and perform in value-based contracts

Included in the Collaborative
Successful Transitions Predictive Models
Predictive models will deliver Cyft SmartLists designed to maximize performance against the following PCF-SIP quality metrics:
- Successful transition of patients to responsible providers, minimizing 90-day admissions or ED visits
- Average length of stay of less than 8 months
- Per capita cost of care lower than the CMS defined benchmarks
- Days at home (applicable in program years 2 through 5)
Learning Community - Using Data to Succeed
The Serious Illness Performance Collaborative participants will use data and lessons learned to support efforts to excel in value-based contracts, including:
- Disseminating best practices and performance improvement strategies
- Participating in multi-site research projects
- Accessing experts in various aspects of technology-enabled serious illness care
- Providing a unified voice by aggregating and presenting evidence to help guide CMS program design
